By completing this form you are indicating your interest in taking part in this program. You will still need to secure your admission into the Early Fall term by paying the $200 deposit.  Once you complete this form and pay the $200 deposit, we will enroll you in the one-credit SLS1501: First Year Experience Seminar. You will be able to attend Fall orientation and if you are receiving an I-20 it will be issued with a start date of Fall.
